When and How is Bone Grafting/Ridge Augmentation Performed?

Prior to or during implant placement, Dr. Marlin frequently performs bone grafting procedures to reinforce implants by facilitating bone growth in extraction sockets (socket preservation), building out an atrophic bony ridge (ridge augmentation), and increasing the bone density (densification). When tooth roots or implants are in the jaw, the bone is preserved. Conversely, when either is not in place, the bone resorbs. Therefore, bone grafting is often needed to rebuild the bone to support the implant.

When the bone has atrophied from disuse, periodontal disease, tooth extraction, or just the individual’s anatomy, an implant should not be placed without bone grafting/ridge augmentation of the site either before or in combination with Precision Implant Placement (see below).

In this example, the tooth position potentially jeopardized the bone when being removed, and the ridge had a 45⁰ angulation while being too narrow (5.6mm) to receive and restore an implant. Therefore, the tooth was precisely extracted, preserving the bone, and bone grafting/ridge augmentation was performed to widen the ridge to 10.4mm while changing its angulation from 45⁰ to 20⁰. By rejuvenating the bone, the implant was now able to be preplanned and inserted to fit in an ideal, rather than a compromised position directly below the temporary crown.

How are Tooth Extractions Best Performed?

We try to save teeth as a priority. However, we may determine that you need a tooth extraction for any number of reasons. Some teeth are extracted because they are severely decayed; others may have advanced periodontal disease or have broken in a way that cannot be repaired.

In most instances, we will extract the hopeless teeth and bone graft the sites to preserve your jawbone. When we remove teeth, we take extra precautions to preserve as much bone as possible and always fill the extraction sockets with a combination of bone and a growth factor that routinely creates very dense bone (even denser than an individual’s own bone).

In the following example, tooth #4 had extreme bone loss (see yellow bracket). The tooth was extracted, and bone plus a growth factor was simultaneously inserted (see yellow arrow). Four months later, after the bone was fully integrated into her jaw, the implant was inserted into newly created, dense bone.

No. With good oral hygiene and regular professional care, your teeth are meant to last a lifetime. However, if left untreated, periodontal (gum) disease can lead to tooth loss. It is the primary cause of tooth loss in adults 35 and over.

Our senior patients may not consider implants due to their ages. From a biologic standpoint, you are never too old to receive dental implant therapy. Our oldest implant patient was ninety-one years old. However, health is more of a factor than age in assessing if a patient is a candidate for dental implants. Implants offer vastly improved chewing and function normally, which is a primary concern of our older patients.

Smoking is a significant risk factor with regard to tooth loss and dental implant failure. Smoking negatively affects blood flow to the bone and tissues surrounding the gums and teeth, which impairs bone healing. People who smoke are at a greater risk of infection or other causes of implant failure following surgery.

We encourage you to kick the habit. There are many new smoking cessation products and support groups to help you. Smoking cessation takes a huge commitment and it is usually easier if you have help. To begin a tobacco cessation program, ask us or your physician for more information.

A mini-implant is a titanium screw with a round head that is placed into your jaw and o-rings are placed into your lower denture to snap your denture securely in place. In many cases these implants can be placed and the denture implants snapped into place the same day. For patients who are interested in preserving their bone (via bone grafting), we use mini implants as a transitional way of having a stable removable, until it can be replaced with the larger implants and a fixed prosthesis.

There are many variables that factor into the cost of high quality, very successful implant therapy (Dental Implant Surgeries and Dental Implant Restoration). Some of them are: how much time and precision is being used in all surgical and restorative procedures and are extra procedures being used to dramatically increase longevity.

“Metal free,” better known as ceramic implants, are another type of implant fixture for restoring missing teeth. Given the huge success rate of titanium implants that we have experienced over the last 35 years, we only use titanium implants with our therapy. Why would we tamper with success?
